Emergency Forms

At the beginning of each school year, parents/guardians must complete a contact/medical concern form to be kept in the division office.  A copy then goes to the clinic for our records.  In case of an emergency or child illness we will contact you or the persons you have listed on this form.  This form is also required for all field trips your child may attend.  You can download the forms through the links below.

If there are medical concerns that school personnel need to be aware of they should be listed on this form. Confidential information that you may not want to include can be discussed with the school nurse. 

It is very important we have an emergency form for every student. If you have more than one child attending TBS, it is required to have a separate form for each child. 

You must keep this form updated!  If you or your contacts move or change phone numbers at home or work during the school year, please email the office of the school division your child attends by clicking on the following link/s:  Lower School Office, Middle School Office or Upper School Office.
