Immunizations

Florida immunization regulations specify the requirements for entry into school. Students can be excluded from school if they do not meet these requirements.

A new physical exam and immunization records are required upon entering,  Pre-k, K and 7th grade and for all new students entering The Benjamin School, regardless of their grade. 

The Florida Certification of Immunization (blue form) and the State of Florida School Entry Health Exam (yellow form) can be obtained from your pediatrician or the Health Department at the time of your child's physical.
